Form Builder: rules and policies

1. What Form Builder is

Form Builder makes a ServiceM8 form for your business. You describe the form, or you upload the form you use now. An AI designer builds it. You see every page and the price before you pay. After payment you download the form and import it in ServiceM8.

Form Builder is a service of WorkM8 Consulting. It is not made by ServiceM8.

2. What you need

• A ServiceM8 account for your business.
• To connect that account once ("Connect ServiceM8"). You log in to ServiceM8 and approve Form Builder. Form Builder reads the name of your business, its email address, its region setting and the id of your account. It does not read your jobs, your clients or your staff, and it keeps no key to your ServiceM8 account.
• A description of the form you want, or a file of it: a PDF, a photo, or a form file from ServiceM8 (.sm8f).

3. Before you pay

These are free:

• Connect, pick a look, add your logo and your colour.
• Have the form built, and ask for changes in the chat.
• See every page of the form.
• Try the form with sample answers, or run it one question at a time, as a technician will.

The pages you see before payment have a watermark. You cannot download the form before payment.

Limits before payment:

• You can have 5 forms that are not paid for at one time.
• A form that is not paid for, and that has no change for 30 days, is removed.
• A form can have up to 25 chat messages before payment.
• There is a fair-use limit on design time for each account. When you reach it, the chat tells you to contact us.
• On a very busy day, the designer can be closed for new work until the next day. Your work is saved. A form that you have paid for is not held up.

4. Price

Every price is in Australian dollars. For a customer in Australia, the price includes GST. For a customer in another country, no GST is added. The price you see is the price you pay.

A form is $249. This includes:

• up to 25 questions,
• up to 5 rules (a rule shows a question only for some answers),
• up to 2 printed pages,
• up to 8 tables and 4 photos,
• the look, your logo, your colour, and tick and cross icons for answers.

A larger form costs more. You see each line of the price beside your form, as soon as there is a form.

A form over $1,190 is larger than we sell online. Ask us for a quote: press "Make enquiry" under the price, or contact us. An enquiry costs nothing, and you do not have to accept the quote.

The designer does not give discounts, and it does not make a form smaller to reach a lower price unless you ask.

5. What you get

• One form file for ServiceM8.
• To use it: in ServiceM8, open Settings, then Forms, then Import, and choose the file.
• The form prints the business name of the ServiceM8 account that you connected.
• Your logo: the one you added, or, with none, the logo that is set in your ServiceM8 account (ServiceM8 puts it in when you import the form).

6. Changes after you buy

A purchase includes 5 changes to that form.

• A change is a changed form that you download. A chat message is not a change.
• A new name for the form is a change.
• For 30 days after the purchase, a change is free.
• After 30 days, a change is $79.
• From the sixth change, a change is $249. That payment is a purchase: it brings 5 changes again. Or you can edit the form yourself in Form Designer (part 13).
• When a change adds an extra from the price list, you pay the price of that extra.
• You can download the version you bought again, at no cost.
• Each payment for a form brings 25 chat messages for it again. A free change does not.

A change must be the same form. It must keep at least 70% of the questions that you bought. Each of those questions must still be asked and must still print. With fewer, it is a new form, at the price of a new form.

7. Your form is for your business

The form is made for one ServiceM8 account: the one you connected.

• It prints only in that account.
• It cannot be changed in ServiceM8's own form editor.
• It cannot be exported from ServiceM8.
• The business details on it are always those of that account. Form Builder does not put another business's name, address or phone number on a form.

You can use the form on every job in your account, for as long as you want. You cannot give it or sell it to another business. To change it, use Form Builder (part 6) or Form Designer (part 13).

8. Keep your own copy

You must keep your own copy of each form that you buy.

• When you download a form, save the file in a place of your own (your computer, your shared drive). Do this for each changed version too.
• Form Builder is not a store for your forms. "My forms" lets you open a form again and download it again, but we do not promise for how long.
• Form Builder does not put the form into ServiceM8 for you. You import the file.
• A form that is imported in ServiceM8 keeps working on your jobs. It does not depend on Form Builder.
• If you lose your file and we no longer have the form, a new form costs the price of a new form.

9. What you upload

You can upload a PDF, a photo (PNG or JPEG) or a form file from ServiceM8 (.sm8f), up to 3 files in one message. A logo is a PNG or JPEG of 1 MB at most.

• Upload only what you have the right to use. Do not upload another business's form as your own.
• Upload an empty form, not a filled-in one. Do not upload the personal details of your clients.
• An uploaded form is the specification. The designer keeps every question, every answer choice and all fixed text. The layout is new and in our own style. It is not a copy of your page.
• A form from ServiceM8 can have up to 120 questions. It becomes a new form in ServiceM8. The answers already on your jobs stay with the old form.
• A form from ServiceM8 with more than 120 questions is not converted here. You can send its file to us from the same window, and we give you a quote for it. That enquiry costs nothing.
• Pictures in your old form do not come over. Your logo is the exception.
• A locked form is refused, because it can be another maker's work. One kind is accepted: a form that Form Builder or Form Designer made for your own account.
• An ABN or a licence number is never copied from an uploaded form. You give yours in the chat.

10. Your part: check the form

An AI builds the form. It can make mistakes.

• Before you pay, read every page, and run the form with different answers.
• After you import it, try it on a test job before your technicians use it.
• The words on the form are yours. Form Builder gives no legal, safety or compliance advice. A safety form, a certificate or a set of conditions made here is not checked against any law, standard or regulation. That check is yours.

11. Your information

What we keep:

• the name of your business, its email address, its region setting and the id of your ServiceM8 account,
• your chat messages, the files and the logo you upload, and your forms,
• a record of each purchase: the form, the date and the amount,
• a record of the steps you take here (for example: connected, form built, paid), with the name, the email address and the region of your business. Nothing of your forms, your chat or your files is in it.
• when you make an enquiry: your form (for a form from ServiceM8 that is too large to convert: the file you uploaded), and the name, email address, phone number, address and message that you type for it.

Where it is:

• On WorkM8 Consulting's server in Sydney, Australia, with backups made by the hosting company.
• Your chat messages and uploaded files are sent to the AI service that builds the form: Claude, from Anthropic, a company in the United States. They can be processed outside Australia. Under its terms with us, Anthropic may not train its AI models on your content.
• The record of the steps you take is kept in WorkM8 Consulting's own usage system, which runs at Cloudflare and can be outside Australia.
• An enquiry goes to WorkM8 Consulting by email, with your form. It is sent through an email service (Resend, a company in the United States) and is kept in WorkM8 Consulting's mailbox and in its job system (ServiceM8).
• Payment is made on Stripe's page. We do not see or keep your card details.

What we do with it: we build your form, we show it to you again, we answer your questions, and we see how the service is used, to make it better. We do not sell it. We do not use your form for another customer.

The site uses cookies only to sign you in and to keep you signed in. It has no advertising or tracking cookies.

You can remove a form that is not paid for, in "My forms". To have a paid form or your account's information removed, contact us.

12. Payment and refunds

• You pay once for each purchase, on Stripe's secure page. There is no subscription.
• You pay after you have seen every page of the form and its price.
• Before you pay, you accept the WorkM8 Software & Services Agreement (https://workm8consulting.com/agreement) and these rules. Where the two differ for Form Builder, these rules apply.
• A form is made to your order, and you see all of it before you pay. For this reason a payment is not refundable, except where the law requires it (this includes the Australian Consumer Law), or when we decide to give a refund.
• If the form you downloaded has a fault (it does not import in ServiceM8, or it is not the form you saw), tell us and we will correct it.
• Nothing in these rules takes away a right you have under the Australian Consumer Law.

13. Form Designer

Form Designer is WorkM8 Consulting's ServiceM8 add-on for people who want to design and change forms themselves.

• A form you buy in Form Builder is free to edit in Form Designer. You need no plan, and the form gets no "Made with Form Designer" line. The 70% rule of part 6 applies there too.
• Form Designer can cost less. There, one of eight ready-made forms costs $149, and a yearly plan covers every form you make. If one of those forms suits you, or you want to design forms yourself, look at Form Designer first: https://workm8consulting.com/form-designer
• A form that you made or changed in Form Designer can be uploaded to Form Builder for more work by the AI. It is then a new Form Builder form, at the price of a new form.
